Transitioning from Short-Term Rehabilitation to Long-Term Care

After completing Regent’s restorative therapy program, many people are able to safely move back home. We work with patients every step of the way in order to help promote a smooth transition from rehabilitation back into their community. If required, we can also provide ample information regarding home care and can recommend a number of trusted, friendly and proficient home care services.

If patients still require 24-hour skilled nursing care, they can transition seamlessly to long-term care at Regent Park. Our conscientious staff will extend your care plan and continue to provide assessments, evaluations, maintenance therapy and any other treatments you might require throughout your stay.
